Do I need a new physical for every sport?
No, physicals are good for 1 year from the date the date of exam. Health History Update is a part of the registration you completed on the Genesis parent portal. You are only submitting the Medical Eligibility Form once to the school, your physician completes this form.
If my student played a fall sport, do I need to fill anything out for Winter or Spring?
As long as your child's physical was within 365 days of the start of his/her sport and the Heath History Update form was completed and dated within 365 days of the start of his/her sport, you only need to complete and update the online sport registration in the Genesis parent portal. If you have a physical on file, you can see the date of exam on the Genesis parent portal under the athletics tab.
Who fills out the form?
Parent/Guardians must complete and sign the Health History Form within 365 days of the sport's start. Your Physician completes the state Sports Physical Form. The doctor completes the Medical Eligibility Form and you turn this in to your school nurse.
What happens if the doctor forgets to sign or fill out the form completely?
Your forms will be returned to you for completion. We cannot accept incomplete forms. No faxing, or emailing these forms.

Genesis Sports Registration Instructions for step-by-step instructions on how to register your student (s) for a sport.
