Do I need a new physical for every sport?
No, physicals are good for 1 year from the date of exam. The Health History Update is now a part of the sport registration you complete on the Genesis parent portal. You are only submitting the
Medical Eligibility Form once to the school, your physician completes this form.
If my student played a fall sport, do I need to fill anything out for Winter or Spring?
As long as your child's physical was completed within 365 days of the start of his/her sport, you only need to complete the online Sports Registration in the Genesis parent portal (this now includes the health history update). If you have a physical on file, you can see the date of exam on the Genesis parent portal under the athletics tab.
Who fills out the form?
Parent/Guardians must complete and sign the Health History Form within 365 days of the sport's start. Your Physician completes the state Sports Physical Form. The doctor completes the Medical Eligibility Form and you turn this in to your school nurse. No faxing or emails, you must drop off the original form.
What happens if the doctor forgets to sign or fill out the form completely?
Your forms will be returned to you for completion. We cannot accept incomplete forms. No faxing, or emailing these forms.
