Reporting Absences

Parents may create Attendance Notes in the Genesis Parent Portal which will function as the parent written note to request an excused absence, late arrival or early dismissal. To create a parent attendance note in Genesis Parent Portal, click on the Attendance tab followed by the Notify Attendance Office tab.

Homework request for absent students who are out of school more than one consecutive day should call the Guidance Office at 856-778-6620 ext. 13170.

SPORTS
How to register for sports at WAMS.
Go to our Sports Online Registration (OR) link. Note: You must register online and submit Physical and History forms to the nurse. ** You can not upload physicals, fax, or email! Original documents must be turned in to the health office.  

On-line Registration for SPRING Sports is Now Open.


Completing Sports Paperwork:
Deadline for Spring Paperwork is Friday, February 16, 2024.


Parents complete the Pre-Participation Health History Form and physicians must complete the Physical Form, pages 3 & 4 in the Athletic Sports Physical and Health History Update Form. All signatures must be completed. Physicals are good for one year. The physical must be in force on the first day of practice, otherwise a new physical shall be needed.

You must use the State Physical Forms, linked above, no exceptions. Please make sure the date of the physical exam is clearly labeled (this may be different from the day the Physician signs the form). The Physician must fill out the form completely including blood pressure, pulse, and vision (common areas they tend to leave blank). If there are missing areas, forms will be returned to the parent.

As long as your child's physical was within 365 days of the start of his/her sport and the Heath History Update form was completed and dated within 365 days of the start of his/her sport, you only need to complete and update the online sport registration in the Genesis parent portal.



If the Physical Form is not completely filled out, your forms will be returned to you causing delays. We can not process incomplete forms. NO EXCEPTIONS.




The MHS School Doctor strongly encourages all potential student-athletes to have their sport physicals administered by their family doctors.

The Moorestown Preparticipation Physical Packet is mandatory for participation in interscholastic sports if your current physical was administered more than 365 days before the start of your Fall Sport’s first practice.


 


Records to Keep at Home

It's a good idea to keep records of the following health information:

* Any health screening results, such as those for vision, hearing, and dental.
* Records of any major health problems including broken bones, surgeries or hospital stays.
* A list of medicines your child has used in the past or is currently taking. Include prescription and over-the-counter medicines, dietary and herbal supplements, and vitamins and minerals.
* A list of allergies including to any food and/or drugs.

It's also a good idea to include:

* Your Pharmacy name and phone number.
* The poison control phone number.
* Records of insurance claims and payments.
* Written notes from your child's doctor or doctor visits.
* Anything else about your child's health that you think is important.
