Reporting Absences

To report a student absent, you must call the nurse's office Absence Line 856-778-6620 ext.13041.
This attendance line is open 24 hours a day, please feel free to leave a message anytime. We are unable to accept email to report your student absent.
Homework request for absent students who are out of school more than one consecutive day should call the Guidance Office at 856-778-6620 ext. 13170.


Welcome to the WAMS Health Office

Fall Sports Physical and Health Forms Due by 8/3/18
During the summer, there is a drop box in the main office to turn in all Physical and History Forms for the Fall Sports Season. (If you are not playing a fall sport, you do not need to turn in forms at this time.)

Sports Physical Forms can not be processed unless they are 100% complete (this includes vital signs, height, weight, vision screen, and date of exam). NO EXCEPTIONS

Go to our Sports Online Registration (OR) link. Note: You must register online and then complete the Physical and History forms.

Completing Sports Paperwork:

Parents complete the Health History Form, Physicians must complete the entire Physical Form.

Physical Form completed by the Physician is good for ONE YEAR and must use the State Physical Form found here, no exceptions. Please make sure the date of the physical exam is clearly labeled (this may be different from the day the Physician signs the form). The Physician must fill out the form completely including blood pressure, pulse, and vision (common areas they tend to leave blank). If the Physical Form is not completely filled out, your forms will be returned to you causing delays. We can not process incomplete forms. 

Health History Form is updated for each sport, dated within 90 days of the start of the sport. 

 


Records to Keep at Home

It's a good idea to keep records of the following health information:

* Any health screening results, such as those for vision, hearing, and dental.
* Records of any major health problems including broken bones, surgeries or hospital stays.
* A list of medicines your child has used in the past or is currently taking. Include prescription and over-the-counter medicines, dietary and herbal supplements, and vitamins and minerals.
* A list of allergies including to any food and/or drugs.

It's also a good idea to include:

* Your Pharmacy name and phone number.
* The poison control phone number.
* Records of insurance claims and payments.
* Written notes from your child's doctor or doctor visits.
* Anything else about your child's health that you think is important.
