Reporting Absences
Parents may create Attendance Notes in the Genesis Parent Portal which will function as the parent written note to request an excused absence, late arrival or early dismissal. To create a parent attendance note in Genesis Parent Portal, click on the Attendance tab followed by the Notify Attendance Office tab.

Homework request for absent students who are out of school more than one consecutive day should call the Guidance Office at 856-778-6620 ext. 13170.

SPORTS
You must register online and submit the Medical Eligibility Form to the nurse. ** You can not upload physicals, fax, or email! Original documents must be turned in to the health office.

PLEASE READ ALL THE INFORMATION BELOW CAREFULLY
The New Jersey Department of
Education (NJDOE) has changed the sports physical submission process. The sports physical examination form and the health history form no longer needs to be submitted to the school. YOUR Physician who performs the exam keeps them at his office. This is an update to NJ state law that requires all students that plan to participate in the after-school Athletic Program in Middle School and High School.
***The ONLY form you submit to your school nurse is THE MEDICAL ELIGIBILITY FORM.
Your doctor completes this form when he does the physical.
You must submit the original, no faxes or emails.***
- Preparticipation Physical Exams are valid for 365 days. The physical must be current on the day of the first practice to be considered valid. Please be aware that physical exams are recorded based on the date of the exam, not the date the form was completed by the physician.
- Sports Physical Exams must be recorded on the New Jersey Department of Education Annual Athletic Pre-Participation Evaluation Form. THIS FORM IS TO BE KEPT ON FILE BY THE STUDENT’S HEALTHCARE PROVIDER.
- THE MEDICAL ELIGIBILITY FORM MUST BE COMPLETED BY THE HEALTHCARE PROVIDER, AND SUBMITTED TO THE SCHOOL DISTRICT. THIS IS THE ONLY FORM THAT IS REQUIRED TO BE SUBMITTED TO THE NURSE’S OFFICE.
- NO student is permitted to try-out or compete in a sport without the documentation being completed. Registration will open open over the summer for FALL sports.
- Complete the Health History Update Questionnaire in the FORMS SECTION of the Genesis Parent Portal. This is part of the registration process each sport season.
To avoid delays in clearing sports physicals, be sure to turn in your paperwork on time. Please note -No emails or faxes accepted.

Records to Keep at Home
It's a good idea to keep records of the following health information:
* Any health screening results, such as those for vision, hearing, and dental.
* Records of any major health problems including broken bones, surgeries or hospital stays.
* A list of medicines your child has used in the past or is currently taking. Include prescription and over-the-counter medicines, dietary and herbal supplements, and vitamins and minerals.
* A list of allergies including to any food and/or drugs.
It's also a good idea to include:
* Your Pharmacy name and phone number.
* The poison control phone number.
* Records of insurance claims and payments.
* Written notes from your child's doctor or doctor visits.
* Anything else about your child's health that you think is important.
